See a physiotherapist when discomfort is present off the bike, or when it's clearly about your body's capacity rather than the bike.

See a doctor promptly for pain after a crash, severe or worsening pain, night pain, persistent numbness (especially perineal), or any circulation or nerve warning signs. This app will stop and tell you when it sees these patterns.
